Use a heading when writing to your senator or representative. Your heading should have your name, address, e-mail and/or phone number so that a) they know you're an actual constituent and b) they can contact you with a reply. A good example of how to write a letter to your congressman in general, can be found at http://usgovinfo.about.com/od/uscongress/a/letterscongress.htm. Start the actual letter with Dear Congressman, or Dear Senator or Dear Representative, depending on who you're writing to.
Keep your letter brief and factual. If your letter is too long, or based purely on emotion, your congressman may not take it seriously. Try to keep your letter between three and five paragraphs.
